Public walk-in queue (currently closed)

The public online walk-in queue is currently closedand is not collecting entries before launch. When it is later opened by clinic staff, the public queue form will collect only your full name and phone number, and will require you to acknowledge the important notices on the form. That is the minimum information needed to hold your place in line.

The walk-in form does not collect date of birth, Alberta Health Care number, symptoms or reason for visit, medication history, images, or file attachments. A neutral internal reason ("Walk-in queue request") is recorded automatically for staff — it is not a free-text field you fill in.

Your ticket identifier (when the queue is open)

When the walk-in queue is open and you join it, the website will assign a high-entropy ticket identifier and store it locally in your browser so you can reopen your own ticket and cancel it if needed. The identifier is sent to the clinic's hosted backend when you load or cancel your ticket, so the request can be matched to your record.

Submitted information and standard technical logs are processed by the clinic and by contracted hosting and database service providers engaged to operate and secure the service. These vendor relationships remain subject to the clinic's ongoing vendor and privacy review. This notice does not make specific security guarantees, state exact retention periods, or claim certified compliance.

Optional browser notifications

When the queue is open, you may choose to enable browser notifications about your queue position from the ticket page. Notifications are optional; if you do not enable them, you can still return to the ticket page in your browser at any time to check your position.

Please do not send medical information

Do not submit symptoms, diagnoses, medication lists, date of birth, Alberta Health Care numbers, images, files, or clinical history through any public form on this website or by ordinary email. The administrative email is not a secure clinical channel and is not monitored for emergencies.

How to apply as a new patient

The secure online new-patient application is available now at /register. It is a single-page form that collects only the minimum information needed to review an application:

  • full legal name
  • phone number
  • optional email address
  • preferred contact method (phone or email)
  • optional preferred family physician
  • optional preferred days of the week
  • preferred time window (morning, afternoon, evening, or any)
  • required consent and privacy acknowledgements

The online application does not collect date of birth, address, Alberta Health Care number, symptoms, diagnoses, medications, medical history, images, or file attachments. Applicants must not submit medical information through the form or by email — those channels are not secure and are not monitored for emergencies.

Applications may also be initiated by phone at (780) 570-8797, by administrative email (to request a callback or a secure application link only), or in person during regular clinic hours.

Correction, withdrawal, and access

When you submit an online application, the confirmation screen displays a reference code (for example APR-YYYY-XXXXXXXX). To ask about, correct, or withdraw an application you submitted, please call the clinic at (780) 570-8797 and quote your reference code. Please do not email application details or personal health information.

Other patient workflows

Online appointment booking, the public walk-in queue, and the general contact form are not currently available on this website. Because these workflows are disabled, the website does not collect any additional personal information through them.

How information is used

Information submitted through the walk-in queue is used only for the stated administrative purpose — managing the queue and, if needed, contacting you about your place in line. Access is restricted to authorized clinic staff.

Standard server and security logs (such as IP address, request time, and error information) may be processed to operate and secure the website.

Access and correction

You may ask about access to, or correction of, personal information the clinic holds about you by calling the clinic. The detailed process, applicable timelines, retention and deletion practices, and the formally designated Privacy Officer remain subject to ongoing privacy work and have not been finalized. The clinic does not claim certified HIA/PIPA compliance and has not completed a formal Privacy Impact Assessment.

This notice should not be read as a claim of certified HIA/PIPA compliance.
