This website is operated by Sherwood Park Family & Walk-In Clinic(the "Clinic") to share general information about the clinic and related opening and operating information. By using this website you agree to these terms.

What this website currently offers

  • General information about the clinic, physicians, and services.
  • Information about how to apply as a new patient by phone, by administrative email (callback or secure link only), or in person after opening.
  • Information about walk-in care, including a live status area that will show queue information once the queue is opened by clinic staff.

What is not currently available

  • Online appointment booking.
  • The secure online new-patient application form.
  • General contact-form messaging.
  • The public walk-in queue (Closed until manually activated by staff).

Where these features appear in the navigation, they lead to information pages only. Using an information page does not create or confirm an appointment, an application, or a place in a queue. Usual walk-in hours listed on this website do not mean the queue is currently open — the live walk-in status page is the source of current availability.

Walk-in queue (when opened)

  • The public walk-in queue is currently Closed and will remain Closed until manually activated by clinic staff.
  • When it is opened, joining the queue is not an appointment and does not guarantee a same-day assessment.
  • Wait times are estimates. Patients may be seen based on physician availability and clinical priority rather than strict queue order.
  • You will be able to cancel your own place in line from the ticket page in your browser at any time.

New-patient applications

  • The secure online new-patient application is available now at /register. It collects only full legal name, phone number, optional email, preferred contact method, optional preferred family physician, optional preferred days, preferred time window, and required consent and privacy acknowledgements.
  • Applications may also be submitted by phone, by administrative email (to request a callback or a secure application link only), or in person during regular clinic hours.
  • Do not submit symptoms, diagnoses, medications, Alberta Health Care numbers, dates of birth, identification, documents, images, or any other personal health information through the online application, the website, or ordinary email.
  • To request correction or withdrawal of an application you submitted, call the clinic at (780) 570-8797 and quote your application reference code (e.g. APR-YYYY-XXXXXXXX).
  • Submitting an application does not guarantee acceptance, roster placement, a specific physician, a preferred day or time, or an appointment, and does not establish a physician-patient relationship.
  • The clinic has not published a certified HIA/PIPA compliance statement or a completed Privacy Impact Assessment; policies, retention, and vendor review remain subject to ongoing privacy work.

Privacy and local ticket storage

When the public walk-in queue is opened and you join it, the website assigns a high-entropy ticket identifier and stores it locally in your browser. That identifier is sent to the clinic's hosted backend when you load or cancel your ticket. Submitted information and standard technical logs are processed by the clinic and by its contracted hosting and database service providers to operate and secure the service. Lawful use

You agree not to misuse the website, interfere with its operation, or attempt to access areas you are not authorized to use. The Clinic may modify or discontinue features at any time without notice.

Limitation of liability

To the maximum extent permitted by law, the Clinic is not liable for any damages arising from your use of this website. The website is provided on an "as is" basis without warranties of any kind.
